HKFM is required by law to maintain the privacy of Protected Health Information ("PHI"), provide this Notice, and follow its terms.

 

How We May Use and Disclose PHI

  • Treatment: to provide and coordinate care

  • Payment: to bill and collect for services

  • Healthcare Operations: quality improvement, training, business operations

  • As Required by Law: certain public health and legal disclosures

 

Your Rights

  • Request access to your records

  • Request amendments/corrections

  • Request restrictions (in some cases)

  • Request confidential communications

  • Receive an accounting of certain disclosures

  • Receive a paper copy of this Notice

  • File a complaint without retaliation
