No Provider–Patient Relationship

Use of this Site does not create a provider–patient relationship with HKFM or any of its providers, including Amy Patton, DNP, CPNP-PC, FMACP. A provider–patient relationship is formed only after required intake, signed consents, and formal acceptance as a patient by HKFM.
 

No Emergency Services

The Site is not for emergencies. If you believe you have an emergency, call 911 or seek immediate emergency care.
